Contractor Accident and Incident Reporting and Investigation

Reporting:

A written notification of a contractor accident or incident must be made to the Xerox Contract Administrator and the Contractor Safety Engineer within 24 hours of the accident or incident.

Investigation:

As soon as practical, no later than 14 days, an accident investigation must be conducted by the contractor and documented. The documentation must describe the incident and identify the cause(s) and the corrective action(s) which will prevent future incidents. Copies of the investigation report should be sent to the Xerox Contract Administrator and to the Contractor Safety Engineer in Building 205. All accidents are to be reported regardless of whether they result in injury.
